The Transition & Closing Phase consists of completing final details for finishing out the project.  The Project Manager will work to resolve any final project details, and obtain sponsor agreement on the closing of the project.  The PM will conduct an After-Action Review to obtain lessons learned, record information about areas for improvement, and document best practices.  The PM will also make any final updates to the Project Plan.  Finally, the PM will archive all appropriate project artifacts according to the organization's Standards & Conventions.

Objectives of the Transition & Closing Phase are:
1.  Evaluate and report project accomplishments, deliverables, best practices, and lessons learned
2.  Reconcile and close budgets, contracts, etc.
3.  Transition project staff to other projects or back to operations
4.  Share project knowledge gained with sponsor and other stakeholders/groups

Outputs of the Transition & Closing Phase are:
1.  Completed and approved After-Action Report
2.  Additional, more detailed Lessons Learned (optional)
3.  Customer Satisfaction Survey (optional)
4.  Final, reconciled Project Artifacts (all project documents, financials, contracts, etc.)
